Preventive Care
Improve the efficacy and economics of patient care to make it easier for caregivers to keep their patients healthy. Objective data about the patient’s health can be gathered daily and specifically for a visit with the caregiver. These data can help the caregiver to develop better care plans for the patient and to monitor the patient between telehealth visits and office visits. Early detection and action on health issues can keep the patient healthy and minimize expensive episodes and procedures.

Transitional Care
Keep patients engaged on their health recovery through outcome focused engagement, education and support. About 15% – 25% of the time, patients get readmitted to hospitals following a procedure. We will support the engagement of the patient prior to and following high-risk procedures to ensure successful procedures and successful recovery.

Personalized Care
Empower better health outcomes and decision-making for both patients and caregivers through targeted and timely monitoring, insights, and education. Patients will be engaged to monitor their health data and will be made aware of steps they can take to maintain good health and avoid unnecessary health risks.
